INFORMATION ON CITIZENSHIP AND POLISH PASSPORT

The General Consulate of Poland in Curitiba informs that, to get the confirmation of the Polish citizenship and, in consequence to request the Polish passport, it is done necessary the presentation of the following documents:

1. a " kwestionariusz paszportowy " carefully filled out in Pole and signed by the interested person;

2. two copies of the resumé written in Poles and signed by the interested person;

3. Three pictures 4X5 cm recent with name in the verse; colored or black / white; done in paper of possible finer thickness; of half left profile (without glasses); 

4. One of the documents of the interested or of one of your country emitted by the Polish authorities:

5. Polish passport (paszport)

6. confirmation of the Polish citizenship (poswadczenie obywatelstwa polskiego);

7. identity card (dowod osobisty);

8. Extract of the civil registration (wiciag z ksiag ludnoscistalej);

9. Authenticated Xerox of the identity card or of the bill of foreigner's identity emitted by the Brazilian authorities. The naturalization was happened in the family to supply the authenticated Xerox of the certificate of naturalization of the interested or of the direct antecedent;
